24109. Anne Poole
Anne has helped me more than words can express. She has been there for me when ive left suicidal and talked sense into me. She also has given me help and support where my body needs it. She has got me help to keep me fit and my feet healthy. She is a star and id nominate her above anyone else in Britain. If i was her boss id give her a massive pay rise also. I would love her to win an award, i would not be here if it was not for anne. She has already won an award in my eyes. She is an angel and won a place in my life forever. I would also like to add that Anne Poole has even offered to come out and see me on the spot where i was suffering a severe panic attack and crying on the phone,she said i will come out straight away, and that to me is the nicest and kindest thing anyone has done for me, as i have no family and nobody to turn to yet anne was willing to be there for me. I wish i could nominate her for a heart of gold award if there was one as i firmly believe she deserves it. Also if not for Anne, I may of lost my feet by now, she managed to get me a package set up where i now have people coming in and washing my feet and cleaning and drying them as i have diabetes and they got infected once, but now i dont have to worry so much and its all because of what anne has done. If I was boss of the social services i would promote anne, she would be in a higher job and on more money. I know money is tight everywhere, but anne deserves every penny she earns and if ever redundancies ever come about and anne was one, and id won the lottery id offer to pay her wages so she could stay in a job which she clearly enjoys, and also which she gets help for the needy and helps to put their minds at ease.

I have known Emma Jepson over 2 years now and would like to say i wouldnt be where I am today if it wasnt for Emma. She has helped me and my daughter with so much especially with Tegans statementing. I find Emma very easy to get on with and really pleasant at all times. Couldnt thank Emma enough for all the hard work shes done.

Because I had a brain tumour followed by a heart attack, I am now in my late 40s, so not that old. It left me with very little eye sight, mobility, few friends and very depressed and lonely. They helped to re-open my eyes to reading with extra large print. I can only manage a page a day, but it makes the books exciting!!!! They were kind, patient and most of all interested in me and what way they could help and went out of their way to order in 3 of my favourite authors books from all over the region. Apart from improving my leisure time, they did something to restore my confidence, so they deserve this for sure!!!

With respect to the care and support given to my mum Mrs Violet Clarke - supporting her in adjusting to bereavement and helping to rebuild her life, despite being newly diagnosed with dementia. Extremely patient and positive. Also extending her support to me and my sister. - I cannot speak highly enough of the service we as a family have recieved from Stoke-on-Trent Social Services during the recent trauma of my dads death and mums diagnosis very soon after, of dementia. But these three people especially deserve to have their dedication, professionalism and extra caring approach recognised as for us they went well beyond the "extra mile". Dave first became involved when dad was in hospital recovering from a fall bit it had become apparent that he would need care at home upon discharge. Dave was able to gain the confidence and trust of my fiercly independent dad and gently break through their barriers to accepting help. Sadly dad didnt make it home, he passed away in September 2011, then shortly afterwards my devastated mum was diagnosed with dementia. Dave and the team have been my rock through all this. Dementia is a terrifying illness. Although ive nursed many patients myself with dementia, when its your own family its totally different. Dave has guided us through the sorting out of appropriate care and support for mum. Through Dave, Helen and Robs thorough patient care we have been able to see things in a much more positive light. Their sensitive inobtrusive support has given my mum the confidence to remain in her home of 58 years and take encouraging steps towards rebuilding her life. Dave has also ensured that every possible support for mums safety has been put in place which gives me and my sister a great deal of reassurance. Mum is of course the "client" but Dave and the team have extended their support to me and my sister, directing us to support for ourselves even though neither of us live in Stoke-on-Trent now. Nothing is too much trouble, when we have to ring with our concerns. I work in the community and am so aware of the constraints on services these days but in spite of all this, Dave and his team have managed to put together an amazing care package to support mum, in every way respectful of her dignity, privacy and her own particular needs. Dave, Helen and Rob and many others with them are a credit to the City of Stoke-on-Trent, and make me still proud to say that my home town is Stoke-on-Trent even after some 27 years district nursing in Nottingham! Please nominate Dave, Helen and Rob for this wow award and pass on my sincere thanks for all that they have done to support us all.

I have recently been cared for by the stay at home team after a stay in hospital. Although all the carers in the team have done a good job Josie deserves this award as she know that extra concern for my needs. She never rushes me, does her job expertly and always gives time to sit and talk to me like a friend and at 85 i am grateful for such good company. A lovely lady!
