The next steps are easy - simply provide your name, and email address or phone number together with your nomination in the boxes below. Your name will appear alongside your nomination. All other details will not be used for any other purpose other than to contact you to let you know whether your nomination is successful.

How to nominate

To nominate a member of staff from Medway NHS Foundation Trust there are just four criteria that must be met:

  1. The nomination has to be something that makes people go ‘WOW!’ due to first class patient experience.
  2. The nomination needs to be about a recent experience not something that took place a year ago.
  3. It must be about a genuine patient experience, something that has happened to you personally.
  4. You cannot nominate a member of staff from Medway NHS Foundation Trust if you have a personal involvement with them.
