The next steps are easy - simply provide your name, and email address or phone number together with your nomination in the boxes below. Your name will appear alongside your nomination. All other details will not be used for any other purpose other than to contact you to let you know whether your nomination is successful.
To nominate a Burton Hospitals employee there are just three criteria that must be met:
- The nomination has to be something that makes people go 'WOW!' due to first class customer service
- The nomination needs to be about a recent experience not something that took place a year ago
- It must be about a genuine experience, something that has happened to you personally

All we require is your name, email address and phone number, so we can confirm your nomination and let you know if we get an award, and finally the reason for nominating them for this award.
